I believe the issue was a miss-configured network or System Manager permissions for accessing the Cloud9 instance.
I recently went through the exercise of connecting to an EC2 instance over SSM. After that worked successfully, I tried another instance of Cloud9, paying attention to use the AZs that aligned to the SSM endpoints and now I see the entries.
It's strange that it allowed me to connect to the Cloud9 instance at all since I always choose SSM for the Cloud9 connectivity, but that did seem to be the issue.

I just created and checked Cloud9 in my environment.
In my environment it is possible to choose.
