Can the QnA Bot be used to carry out surveys? I need to collect an answer from the user. I want to define up to 5 possible answers for the user to choose from.

Can the QnA Bot also randomly select questions from a question bank so the user does not know what question they will face each time they fill in the survey?

Yes based on your business case and logic, you can use a QnA bot for questions using quizzes, polls, surveys, and tests
