CTRs not generated for contactids that are available in the ATR files

Hi Team,
I've come across missing CTRs for contact ids.
An example scenario is a failed outbound call where the reason for failure could've been something on the agent workstation end (from the examples I've seen).
In the ATR data or the historical agent audit, I can see that a contact id was generated by Connect in this case, but when I click into the contact id from the agent audit screen, it shows me a 404 error page and I cannot find that same contact id via contact search or by querying the CTRs in S3.
Shouldn't all contact ids generated by Connect have a CTR record ?
Is this a known issue?
