ACCESS_DENIED_CONTACT_SUPPORT in Notify

I'm trying to set up AWS notify for SMS-based MFA codes. I created a notify configuration to try to test everything with, but my config gets rejected with "ACCESS_DENIED_CONTACT_SUPPORT" as the rejection reason. I'm wondering if the automated review doesn't like the name of my app? That's the only free-text thing in the config; I can't imagine I got rejected for choosing "English" or template-004. I tried a couple different variations on the name. Always the same thing: ACCESS_DENIED_CONTACT_SUPPORT, so I opened a support case.

It's been a little over a month since I opened the case and it's still unassigned. I'm not in a rush particularly, but I'm kind-of starting to loose hope that anyone is going to pick up my ticket.
