When this kind of problem arises, incorrect file and folder permissions are usually to blame. For WordPress to install, update, and manage plugins, certain permissions are needed. Proper file operations can be prevented by mismatched file/folder ownership between the webserver user (such as www-data or apache) and the user you use to access via FTP.

It seemed like getting rid of Cloudflare seemed to have resolved the issue. While I know they should be compatible it seems like maybe this was a combination of permissions and caching issues.
